Where: Mechanicville Jr./Sr. High School Cafeteria
When: Friday, November 19th
Time: 3pm-8pm
Provider: Adirondack Health and Wellness
Vaccine: Pfizer

If you are interested in participating, follow the following steps…
Step 1 – Sign up using this link to reserve a time on 11/19/2021: https://booknow.appointment-
Step 2 – Click the link to access the NYS form that parents are required to fill out for the clinic: https://forms.ny.gov/s3/
Upon completion of this form, parents must print the completion page OR screen shot/take photo of it to bring to the clinic
Step 3 – Print and complete the “Parent Consent Form” that can be found via the following link to bring with you the day of the clinic – copies are also available in the main offices at each building: https://tinyurl.com/bfa83cpt
Step 4 – Be sure to bring proof of insurance to the clinic the day of administration
…..to access the clinic, please park in the main lot of the Jr./Sr. High School and enter the building through the doors under the red canopy – follow the hallway to your right to access the cafeteria (site of the clinic)
