Vaccines

Diphtheria and Tetanus toxoid-containing vaccine and Pertussis vaccine (DTaP/DTP/Tdap/Td)

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): 4 doses
  • Kindergarten and Grades 1, 2, 3, 4 and 5: 5 doses or 4 doses if the 4th dose was received at 4 years or older or 3 doses if 7 years or older and the series was started at 1 year or older
  • Grades 6, 7, 8, 9, 10and 11: 3 doses
  • Grade 12: 3 doses

Tetanus and Diphtheria toxoid-containing vaccine and Pertussis vaccine adolescent booster (Tdap)

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): Not applicable
  • Kindergarten and Grades 1, 2, 3, 4 and 5: Not applicable
  • Grades 6, 7, 8, 9, 10and 11: 1 dose
  • Grade 12: 1 dose

Polio vaccine (IPV/OPV)

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): 3 doses
  • Kindergarten and Grades 1, 2, 3, 4 and 5: 4 doses or 3 doses if the 3rd dose was received at 4 years or older
  • Grades 6, 7, 8, 9, 10and 11: 4 doses or 3 doses if the 3rd dose was received at 4 years or older
  • Grade 12: 4 doses or 3 doses if the 3rd dose was received at 4 years or older

Measles, Mumps and Rubella vaccine (MMR)

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): 1 dose
  • Kindergarten and Grades 1, 2, 3, 4 and 5: 2 doses
  • Grades 6, 7, 8, 9, 10and 11: 2 doses
  • Grade 12: 2 doses

Hepatitis B vaccine

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): 3 doses
  • Kindergarten and Grades 1, 2, 3, 4 and 5: 3 doses or 2 doses of adult hepatitis B vaccine (Recombivax) for children who received the doses at least 4 months apart between the ages of 11 through 15 years
  • Grades 6, 7, 8, 9, 10and 11: 3 doses or 2 doses of adult hepatitis B vaccine (Recombivax) for children who received the doses at least 4 months apart between the ages of 11 through 15 years
  • Grade 12: 3 doses or 2 doses of adult hepatitis B vaccine (Recombivax) for children who received the doses at least 4 months apart between the ages of 11 through 15 years

Varicella (Chickenpox) vaccine

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): 1 dose
  • Kindergarten and Grades 1, 2, 3, 4 and 5: 2 doses
  • Grades 6, 7, 8, 9, 10and 11: 2 doses
  • Grade 12: 2 doses

Meningococcal conjugate vaccine (MenACWY)

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): Not applicable
  • Kindergarten and Grades 1, 2, 3, 4 and 5: Not applicable
  • Grades 6, 7, 8, 9, 10and 11: Grades 7, 8, 9, 10 and 11: 1 dose
  • Grade 12: 2 doses or 1 dose if the dose was received at 16 years or older

Haemophilus influenzae type b conjugate vaccine (Hib)

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): 1 to 4 doses
  • Kindergarten and Grades 1, 2, 3, 4 and 5: Not applicable
  • Grades 6, 7, 8, 9, 10and 11: Not applicable

Pneumococcal Conjugate vaccine (PCV)

  • Prekindergarten(Day Care, Head Start, Nursery or Pre-k): 1 to 4 doses
  • Kindergarten and Grades 1, 2, 3, 4 and 5: Not applicable
  • Grades 6, 7, 8, 9, 10and 11: Not applicable
  • Grade 12: Not applicable

Student Physicals

Medication Policy

Education law is very strict in the control of over the counter and prescription drugs; therefore, we ask all families involved to follow this outline. Most medications can be given outside of the school hours. Please ask your doctor to schedule as such.

If, however, during the school year it becomes necessary for your child to take medication ordered by a doctor while in school, please adhere to the following rules:

  1. Doctors must fill out and sign a written order.
  2. Parents must fill out and sign a written request.
  3. Medication must be properly labeled from your pharmacy with the patient’s name, dose, name of drug and date.
  4. Parent is to bring the medication into the nurse. Any medications brought in by the student will not be administered.
  5. No student is to have ANY medication with them at school without a prescription on file in the nurse’s office that states “may carry”.

Body Mass Index Notification

New York State requires schools to track each student’s Body Mass Index (BMI) and weight status category as part of school health examinations. Every year, the NYS Department of Health will survey some schools for the number of pupils in each of six possible weight status categories. If the Mechanicville City School District is surveyed by the state, the district will share summary group data only and will not share individual names or information. Parents who wish to exclude their child’s data from such group calculations must contact the child’s school nurse.
