EMPLOYEE NOTIFICATION OF 1095-Cs to COVERED INDIVIDUALS

Covered individuals and full-time employees may receive a copy of their Form 1095-B and 1095-C statements upon request. If you would like a copy of your form, please contact Brianna Patrick at bpatrick@mechanicville.org.
